Do cable/satellite companies pay or get paid by cable channels to get carried?

I was talking with a friend and we didnt know how this works. Do small cable channels (lets say I wanted to start my own cable channel) pay satellite/cable companies to carry them? They make their revenue on advertisements, so it would make sense they would want the most visibility. But on the other hand, it seems like cable/satellite companies have an incentive to carry as many channels as possible, so you would think they would need to pay to get access. I also seem to recall Dish network getting locked out of certain channels because they refused to pay what the channels were asking, but i cant remember for sure if it wasnt the other way around. Anyone know? Maybe its one way for larger channels and the other way around for smaller channels?

Cable and satellite operators are network owner and operator. TV channels pay them for using the infrastructures (cable or satellite links). Usually the cosy is based on capacity or coverage area, and it can be calculated based on the number of subscribers (cost per node) or based on certain technical terms capacities such as cost per megabyte.

Last mile connection (connection between the main cable or main satellite link backbone to the subscribers houses) is usually the responsibility of the TV station.

Satellite and cable owners usually has appoint certain companies to act on their behalf for selling the infrastructures. The lease rates may vary from one provider to another, although they are using the same satellite. This is due to the difference in term of total capacity allocated to each of the companies.

Is it really that difficult for cable and satellite television companies to customize their packages?

All cable companies service have split into separate packages. Sometimes it is necessary to buy 30 additional channels to those who do not want only 2 or 3 that you want. My guess is that they do just so you can buy the premium package. Would it really be so difficult for these companies to offer 30 channels of YOUR choice at a specified price?

That easily could. The problem is that many of the channels charge the cable company per subscriber. By offering on packages so the company cable can spread the cost.
